An interactive environmental health screener built for NYSCHECK, New York State's children's environmental health network. Nine branching questions route a respondent to one of five risk outcome categories, with regional routing built in so results point to the right local resources. Built to embed cleanly into a WordPress/Avada site and to hold up under small-group testing.
